The qualification reflects the role of workers in a range of community settings and clients’ homes, empowering people with disabilities to achieve greater levels of independence, self-reliance, community participation, and wellbeing.
Workers work without direct supervision, may supervise and coordinate a small team, and promote a person-centred approach.
To achieve this qualification, the candidate must complete 120 hours of work as detailed by the Assessment Requirements of the units of competency: https://training.gov.au/Training/Details/CHC43115

As a Service Facilitator, you may be required to conduct in-home face-to-face home support assessments with clients using standardised assessment processes on a daily basis.
You will work together with clients to develop individualized support plans and to match and refer clients to appropriate support services, thereby helping them to achieve wellness and healthy ageing goals.
